Consent Form for Use of Before and After Photos and Testimonials

I, ____________________________________________, hereby grant consent to Tidy Nest Solutions, hereinafter referred to as the "Service Provider," to use my likeness, including photographs and/or written testimonials, for display, promotional, portfolio, or any other purposes as outlined below.

1. Use of Photographs:

• I understand that photographs of my space before and after the organizing project may be taken by the Service Provider.

• I grant permission to the Service Provider to use these photographs for promotional and portfolio purposes, either with or without my name, as specified below:

2. Use of Testimonials:

• I may provide written testimonials regarding my experience with the Service Provider.

• I grant permission to the Service Provider to use these testimonials, either with or without my name, as specified below:

3. Duration of Consent:

• I understand that this consent is valid indefinitely unless I request otherwise in writing.

4. Revocation of Consent:

• I reserve the right to revoke this consent at any time by providing written notice to the Service Provider. Revocation of consent will apply to future use of my likeness, but not to any previous uses.

5. Release and Waiver:

• I release and discharge the Service Provider from any and all claims, demands, or causes of action that may arise out of or relate to the use of my likeness, photographs, or testimonials in accordance with this consent.

6. Confirmation:

• By signing this consent form, I confirm that I have read and understood its contents, and I voluntarily grant consent to the use of my likeness, photographs, and testimonials as described above.
